I think you are right in that the Leviathans are curing cancer to create more food. They had the turduckens to fatten humans up. Now they are healing humans? Less death, fatter humans. I think their plan is to pass as human, eat secretly and privately, and stay on earth. Who wouldn't if they were from purgatory? That's why they want to kill the Winchesters I think. But I think they haven't yet, because they are securing their food source and establishing themselves in society. I don't think they view the Winchesters as a big threat. I think not passing as human was a bigger first threat. Now they are established and we may see more Leviathans. I hope. It's about time.
And the situation with Sam is scary to be sure. I flinched when he fell asleep at the wheel. He's going to crack, and it's obvious. But the only reason Dean didn't notice was because he was bewitched by those darn toe shoes. I think he will notice next episode, the more time he spends with sleepless Sam. HA! We had soulless Sam last season, now we have sleepless Sam.
It all makes me want to tuck Sam into a nice warm bed, with some anti-psychotics while Dean figures out if this is all Supernatural or natural. I hope the haunting by Lucifer is Supernatural. Might mean an easier fix for Dean to figure out.
