I never read the books... or Steven King... actually, I have no idea what you're talking about. :P I DO know, however, that Charlie's aliases are always Steven King Character + Sci-Fi Author... so I'm sure it was done on purpose, whatever it was.
You mean according to Season 9 Sam aye? I didn't get that from his finale speech ... It was all about letting Dean down or Dean thinking he was worthless.
Based on the finale speech too... because even if Sam's decision was that he wanted to complete the trials (and die) in order to make up for letting Dean down all the time, it was still a goal that he decided was worth the cost of his life.
Would it make sense though if Charlie had put Sam there? Because she was worried for Sam too? And she also knew how worried Dean was for Sam? She could have done it subconsciously since it is her dream. Or, maybe once a person is inside a Djinn dream they are part of it too. It automatically changes to incorporate the new addition.

I think if we are going with the dreamroot, killing yourself in the dream would really kill you... like in Dream a Little Dream of Me. I also think that in the happy Djinn dream, you had to make the choice to get out and let go of the wish, whereas with the fear Djinn dream, you had to make the choice to let go of your fear.
